某十四层商住楼，总建筑面积31800.5m2，建筑高度48.55m，全现浇钢筋砼剪力墙结构，筏板式基础。施工过程中，施工单位租赁了一台QTZ200型塔式起重机进行起重吊装作业。2008年5月29日，由于控制大臂连接销轴的止退板的安装违反安装工艺要求，致使止退板失去作用，起重大臂突然折断，将路边10kV高压线砸断，正在邻楼屋面作业的4名民工被砸，造成3人死亡，1人受伤。
经事故调查，该单位对大型机械设备长时间疏于管理，没有建立完整的技术资料档案，也没有向有关部门申请进行技术检验，致使该机械长期处于失控状态。作业人员擅自变更原设计要求，将本应形体锁合的固定方法改变为粘结锁合方式，且又未认真履行监督检查职责，造成这种错误固定方式得不到及时纠正。
